Application Instructions

How to Complete Page 1 of the Application

First Box:

First Name(s) is/are your given name or names and

Last Name is your family name.   If you use more than one family name, please be sure that they are hyphenated (e.g. Marica Kim-Raman)

The program will file your application alphabetically by your family name (the first family name if you use more than one, e.g. under K in the example above).   We will be able to add your letters or recommendation and other documents submitted by others to your file only if they list you in the same way.

If your family name has changed and some of your documentation – such as early transcripts – lists a different name for you, please inform us as to your earlier name so we can watch for materials under more than one family name.  

Second Box:

Place of Birth:  Please list city and country.

Date of arrival to the US (if different):  If you were born in the United States, enter “N/A.”

Date of arrival of parents to the US:    List year – or years – only.

Place of Parents’ birth:   Please list countries of their birth

Places of Family Origins

List the country or area of greatest significance to you first. Identify any specific group or region or area of migration in parentheses.  E.g.: India (Gujarat) or Ukraine (Jewish) or Nigeria (Yoruba) or Palestine,  or Egypt (Coptic).

 If your family was part of a migrant community, you can indicate whether the community or the place of migration was most significant  for you:  eg. Indian (Singapore) or Singapore (Indian); or Chinese (Vietnam) or Vietnam (Chinese).

If your parents had different places of origin, feel free to list a second – or even a third -- country or area, following the same guidelines suggested above.

First Language:   the language you used most growing up as a child. 

Other Languages:  that you learned at home, in classes, or while traveling.  Indicate rough level of competence.

Third Box:

If you were born outside the United States, you must check either the Green Card Holder box or the Naturalized Citizen box and provide the appropriate documentation numbers. 

If you were born in the United States, you must check the Son/Daughter of Naturalized Parents box and provide the citizenship numbers and dates of Naturalization of your parents. 

Do not check more than one box.
